Internship Funding and Payment Forms

In order to receive your summer funding, the following forms must be prepared and delivered to Mariann Adams in Kroon Hall, room 103. Please note that processing can take up to three weeks or more, and checks can NOT be direct deposited.

Please type the forms or write legibly. Please allow ample time for processing.

For US Students

Students must submit the following forms to Mariann:

• Funding Payment Questionnaire
• Assumption of Risk and Waiver: this form is mandatory if interning or conducting research outside of the US.

In Addition to the Above forms:

1. You MUST complete a Curricular Practical Training Request Form after you have been offered your summer internship/research project.
   • Have your Academic Advisor sign the form (Gordon Geballe, Joanne Debernardo or Peter Otis can substitute for your Academic Advisor in signing the form).
   • Take the completed form and your original I-20 form to OISS. OISS can process your form within 24 hours, but it's best if the form is given to OISS several days before needed.
      
2. You MUST meet with Daysi Cardona, International Tax Coordinator, from Yale’s International Tax Office before submitting the below forms to CDO.

3. You MUST submit the following forms to Mariann:
   • Funding Payment Questionnaire
   • Assumption of Risk and Waiver, if interning or researching outside of the US.
   • Vendor Set Up Request (NOTE: you only need to fill out the following: Vendor Legal Name, Social Security Number, Country of Permanent Residence and Mailing Address where you want your check sent)

Please note that awards used in the U.S. by international students are subject to immediate withholding of a percentage of the award.

Vendor Setup Request Form: for International Students ONLY

This form is mandatory for international students in order to receive summer internship funding.  Note: YOU are the "Vendor."
